If you want to become a patient of Woodbury Endocrinology and Diabetes Clinic, we want to make the process simple! On this page, you can find the new patient packet forms needed prior to a consultation so you can print them, fill them out and bring them with you to your appointment. This will save you time and ensure you can gather the necessary information ahead of time. We hope these resources make things easier for you. Please note the following:

• Depending on your healthcare plan (HCP) you may need a referral from your primary care provider (PCP) to see Dr. Alfonso. Contact your HCP directly and, if a referral is needed, ask your PCP for a referral to see Dr. Alfonso with Woodbury Endocrinology and Diabetes Clinic.
• Dr. Alfonso focuses on certain areas of endocrinology. He specifically treats the following medical conditions: diabetes mellitus (type 1, type 2, and gestational diabetes), thyroid disorders (hypothyroidism, hyperthyroidism, thyroid nodules, thyroid cancer), hyperparathyroidism / hypercalcemia, tumors of the pituitary gland, adrenal insufficiency and adrenal adenomas, osteoporosis, and low testosterone in men.
• Although you may self-refer for a consultation with Dr. Alfonso (as allowed by your healthcare plan), all consultation requests are screened to determine if they fall within Dr. Alfonso’s focus areas.
• Dr. Alfonso is a board certified endocrinologist who did his fellowship training at the renowned Walter Reed Army Medical Center in Washington, DC. He is a long-standing member of the Endocrine Society and the American Association of Clinical Endocrinology (AACE). Dr. Alfonso practices evidence-based medicine. He does not practice alternative medicine. Dr. Alfonso follows professional society clinical practice guidelines.
• Dr. Alfonso will work individually with each of his patients and together decide on a treatment plan that best fits a patient’s needs, lifestyle, financial constraints, and personal preference. However, Dr. Alfonso will not treat patients solely based on symptoms while ignoring relevant laboratory or radiological test results that are critical to the patient’s overall wellbeing.
• Dr. Alfonso encourages you to be involved in your own medical care. He will be happy to provide you with educational information from trusted and reputable sources. He also encourages you to become familiar with all aspects of your specific medical condition, and to get a second opinion when needed.
